Death of humans often seen as natural and essential part of life. If anyone has a life insurance policy and he/she expired due to natural, illness or accidental causes then his/her ultimate beneficiary is protected against the financial losses.
Follow these steps to process for your claim.
The claimant must submit written intimation/notification regarding death of policyholder/proposer as soon as possible to initiate the claim process. Intimation/Notification letter should consists of basic information like Policy No., name of insured, date of death, cause of death, place of death, name of nominee/claimant, contact no. and so on. The notification can be given at the nearest branch/sub branch office of the insurance company.
The claimant will be required to provide claimant’s statement (Dabi Bibaran), Original policy bond, Death Registration Certificate, Relationship Certificate, Police Report (Crime scene investigation report and post mortem report), Medical reports, Medical Attendant’s Certificate, Medical Certificate of death from hospital for clam processing. Based on the cause of death, sum at risk, policy duration and other related factor insurance companies may ask for additional documents.
For early claim settlement claimant needs to provide all the required documents as soon as possible. Until and unless claimant provides required documents company cannot take a precise decision.
As per the Life Insurance Claim Guidelines 2076 issued by the insurance regulatory board of Nepal Beema Samiti, the insurer is required to settle a claim within 15 days of receipt of all the required documents including an investigation report sought by the company. The insurance company has the right to accept or decline the respective claim according to the term, conditions and definition of policy and supplementary contract. If the claim required further investigation it has to be done without delay and company can set an example by settling a claim before the deadline of insurance regulatory board.

• Relationship Certificate with proper relations from concerned authority
• Claimant’s Statement (Dabi Bibaran)
• Deceased Identity Certificate to be filled by ward officials
• Citizenship Certificate/Birth Certificate (assured/proposer, claimant/legal guardian)/Marriage Registration Certificate
• Migration Certificate (if migrated from usual place of residence) from concerned authority
• Complete Medical Reports (Admission form, Discharge Summary, Emergency Card/Ticket, OPD Card)
• Death Certificate/Notice of Death from Hospital
• Medical Attendant’s Certificate
• Police Reports (Crime Scene Investigation report including Autopsy report and other related documents)
• Attending Physician Statement
• Total Permanent Disability claim form
• Complete Medical Reports
• Police reports verifying accident
• Medical Reports verifying Critical Illness like Kidney failure/Cancer/Stroke.
• Attending Physician Statement.

Company can setup team of officials to investigate the claim and prepare an investigation report regarding the death of policyholder/proposer but usually concerned branch/sub branch/province office of an insurance company conduct an investigation and arrange for investigation report to settle a claim earlier.
